Federal and state investigations found multiple mistakes at the clinic, including a failure to track her falling vital signs
The New York City medical examiner ruled the official manner of death a "therapeutic complication"
Her daughter, Melissa Rivers, filed a medical malpractice lawsuit against the clinic. The parties later reached a confidential settlement. RIP J
